	Submitting your order form via the internet minimizes errors and provides you with immediate pricing.
	1 - Please ensure that you enter your Group ID Only orders with a valid Group ID will be processed. If you do not have a Group ID, please contact Dental Buy Group to determine if you qualify. 2 - Enter Address information Billing address should match shipping address to facilitate order processing. 3 - Enter the quantity desired to the right of the product If you are using the electronic internet form, all of the prices will be calculated. 4 - Print a copy of the Orderform for your records by clicking on the Print Button. You will also receive a copy of the order with the product shipment. 5 - Press the Send Order button on the bottom of the form to send the order to the Dental Buy Group. Please be patient - it may take 20-30 seconds to submit the form. Once the order is submitted, you will be redirected to a successful confirmation page with additional payment information.

Warranty and
Disclaimer
	Our low pricing reflects our ability to source manufacturers’ original products, and deliver them to you without the added costs of dealer advertizing and markups. We aim to make your buying experience as simple and as satisfying as possible. All our products are manufacturers’ original packaging. We respect all manufacturers’ warranties. All sales are final, but the Dental Buying Group will repair, replace or refund the purchase price of any items with defects in the materials or workmanship. The Dental Buying Group must be notified of any defects within 30 days of receipt to obtain repair, replacement or refund. We do not warranty a product that has been misused, abused or damaged by fire, water or being dropped. The Dental Buying Group makes no warranties of any kind, limited or unlimited, expressed or implied, including warranties of merchantability and fitness for a particular purpose, concerning any of the products, other than those set forth in our printed material. The Dental Buying Group shall not be responsible for any liability, loss, cost, expense or damage to buyer, or to any third party, directly or indirectly, caused by delay in furnishing products or information or any other performances or nonperformances. In no event shall FC Medical Dental Buying Group be liable for special, indirect, incidental or consequential damages of any kind to buyer, or any third party.
Shipments
	An itemized packing list will be included with each order. To ensure proper shipment, compare packing list with items received. If incorrect, the Dental Buy Group must be notified within 5 days of receipt of merchandise. If merchandise is damaged during shipment, notification must be made to the carrier within 5 days. Retain original damaged package for carrier evaluation. Dental Buy Group must also be contacted so that arrangements can be made for replacement or refund.
Payment
	The buyer agrees to pay all “use”, “excise” or sales” taxes or duties resulting from the sale of these items to buyer if required by the province, state or country in which the buyer resides.
Return and
Credit Policy	Authorization must be obtained for all returns by contacting the Dental Buy Group. Items returned must be returned within 30 days upon receipt by customer. We reserve the right to refuse credit on any item returned after 30 days. All products must be returned prepaid and insured at full invoice value.
Price Changes
	In the event of supplier price changes, currency fluctuation or pricing error, the Dental Buy Group reserves the right to contact the purchaser for approval of any price changes before processing the order. If the approval is not granted, a refund for the price changed product will be provided.
Agreement
	By submitting the order to the Dental Buy Group, the customer agrees to the terms and conditions identified on this form.
